This course introduces the Brain Care Score, an evidence-based tool co-developed with patients to assess modifiable risk factors that impact brain and overall health. It will describe how multidisciplinary rehabilitation teams can integrate it into their practice to educate, motivate, and empower patients to make long-term behavior changes. Practical clinical strategies will be provided.

Learning OutcomesFollowing this course, participants will be able to:

  • Identify the components of the Brain Care Score and their relevance to modifiable, lifestyle-related risk factors for brain and functional health.
  • Define the role of the multidisciplinary rehabilitation team in enhancing brain health education and prevention strategies through the Brain Care Score.
  • Identify at least three potential barriers to behavior change and recognize how the Brain Care Score can help provide patient-centered recommendations to mitigate these barriers.
  • Determine at least two actionable steps you can take to integrate the Brain Care Score in your clinical practice for patient education, care planning, and SMART goal setting.

Sydney Malaspina PT, DPT, NCS, CSCS is a board-certified clinical specialist in neurologic physical therapy and practices at Spaulding Outpatient in Medford. She is a mentor in the Boston University Neurologic Physical Therapy Residency and serves as a lab instructor in the BU Doctor of Physical Therapy program. Sydney is also an active fitness coach at CrossFit and Orangetheory Fitness. She earned her Bachelor of Science in Clinical Health Studies (2019) and Doctor of Physical Therapy (2021) from Ithaca College and completed the Boston University Neurologic Physical Therapy Residency in 2023. Her clinical interests include neurologic rehabilitation across the lifespan, with a focus on stroke, brain injury, Parkinson’s disease, balance disorders, vestibular conditions including BPPV, adaptive sports and fitness, and health promotion. Outside of the clinic, she enjoys hiking, running, biking – pretty much anything that is active and outdoors.

Jonathan Rosand, MD, MSC is a physician, scientist, educator, mentor, visionary leader and social entrepreneur dedicated to tackling brain injury treatment and brain disease prevention through large-scale interdisciplinary collaboration. A neurologist with expertise in neurocritical care, cerebrovascular disorders, genetics and epidemiology, he founded Massachusetts General Hospital’s Division of Neurocritical Care, recruiting and mentoring over a dozen faculty members and training nearly a hundred fellows. He holds the J P Kistler Endowed Chair in Neurology at Mass General, is a Professor of Neurology at Harvard Medical School and an Associate Member of the Broad Institute of MIT and Harvard University. In addition to treating patients in the MGH Neuroscience Intensive Care Unit, he maintains an outpatient brain health practice devoted to the care of those recovering from brain injuries as well as those at risk for brain disease.
Jonathan’s research program, launched over 20 years ago, has been a world leader in high-throughput genetic, imaging and epidemiological studies of stroke and the cognitive decline and depression that can come with it. In 2007 Dr Rosand launched the International Stroke Genetics Consortium to bring together the world’s pre-eminent stroke investigators. In 2024 he launched the Global Brain Care Coalition, a learning healthcare network without walls that integrates communities, health and behavioral scientists, private industry, and governments who have committed to an unprecedented vision to prevent brain disease. Estimates are that ~40% of dementia, ~70% of stroke, and at least 35% of depression can be prevented through the lifestyle choices we make every day. The Coalition scales sustained lifestyle and behavioral change through its unparalleled community-engaging, holistic, scientifically grounded, cooperative approach.

Ritika Trehan, MSOT, OTR/L, CBIS is a Clinical Specialist with over 18 years of experience in neurologic rehabilitation across LTACH, skilled nursing, and outpatient settings. She currently practices at Spaulding Outpatient Medford, where she leads complex neurologic and vision rehabilitation for individuals with stroke, concussion, brain injury, neuro-oncology, and neurodegenerative conditions. Ritika is committed to lifelong learning, clinical excellence, and advancing rehabilitation practice. She is passionate about empowering both patients and rehabilitation professionals through knowledge, innovation, and compassionate, evidence-informed care. She has a particular interest in integrating lifestyle-based interventions-including sleep hygiene, energy conservation, fatigue management, and sustainable pacing strategies-into neurologic rehabilitation to optimize long-term brain health and functional
