This course introduces the Brain Care Score, an evidence-based tool co-developed with patients to assess modifiable risk factors that impact brain and overall health. It will describe how multidisciplinary rehabilitation teams can integrate it into their practice to educate, motivate, and empower patients to make long-term behavior changes. Practical clinical strategies will be provided.

Learning Outcomes – Following this course, participants will be able to:
- Identify the components of the Brain Care Score and their relevance to modifiable, lifestyle-related risk factors for brain and functional health.
- Define the role of the multidisciplinary rehabilitation team in enhancing brain health education and prevention strategies through the Brain Care Score.
- Identify at least three potential barriers to behavior change and recognize how the Brain Care Score can help provide patient-centered recommendations to mitigate these barriers.
- Determine at least two actionable steps you can take to integrate the Brain Care Score in your clinical practice for patient education, care planning, and SMART goal setting.

- 30-minutes: Discussion of Recommendations Based on the Results of the Brain Care Score
- 45-minutes: Brain Care Score Use Cases in the Clinical Setting
